To whom have received the letter asking for sending the original passport, need your advice badly!

The letter asked me to fill up and send back an enclosed form in addition to a passport and photos. However, there's no such form included. I'll request them to send me the form for sure. But I want to know what the form is about immediately since I have a very tight timetable.

The letter said "the validity of the visa will be on 11 December 2003". Does it mean " the visa becomes valid only after 11/12"? Isn't this strange? Is someone in similar situation? SIGH
