Is a sublet form a proof of residency for Health card or loan?

When one signs a lease, should the lease incluse all family's name?

Ask the landlord to put all your family members' names in the lease. If only your name there at the beginnign, ask the landlord to make a correction.

should be fine. The fact is you are paying the rent, you should get your name(s) listed in the lease. Ask your landlord to do that.
